Arizona Statewide Independent Living Council (AZSILC) is conducting a Needs Assessment Survey. The goal of this survey is to help assess how programs and services are meeting the needs of people with disabilities in Arizona. 

They want to hear directly from people who have disabilities, their family members, friends, and caregivers. Your feedback will have a direct impact on future services provided in Arizona.

Founded in 1980, Direct was the first Center for Independent Living (CIL) in Arizona. Operated by and for people with disabilities, we serve Cochise, Graham, Greenlee, Pima, and Santa Cruz Counties in Southern Arizona.
